Iran has suffered the heaviest losses, with at least 3,519 people confirmed dead.
According to the human rights group HRANA, 1,598 of the victims were civilians, including 244 children.
The Red Cross and Red Crescent reported that around 19,000 people have been injured across the country.
In Lebanon, Israeli strikes have killed at least 1,318 people, including 124 children. More than 400 fighters from Hezbollah have been killed, along with nine Lebanese soldiers.
Additionally, three Indonesian peacekeepers serving under the United Nations lost their lives.
Iraq has also witnessed rising casualties amid escalating tensions, with 107 people killed, including Iran-aligned militia members, Kurdish fighters, and foreign nationals, according to health officials.
In retaliatory attacks, missile strikes launched from Iran and Lebanon have killed 19 people in Israel. The Israel Defense Forces confirmed that 10 of its soldiers were killed in southern Lebanon.
Separately, an Israeli civilian farmer died near the border due to what authorities described as a military-related incident.
The United States has reported the loss of 13 troops, including fatalities linked to a plane crash in Iraq and combat operations in Iran.
Casualties have also been reported across the wider region, with 11 deaths in the United Arab Emirates, seven each in Qatar and Kuwait, and four each in the West Bank and Syria.
Additional fatalities have been recorded in Bahrain, Oman, and Saudi Arabia, highlighting the widening regional impact of the conflict.
